Professionals Meeting Your Physical, Emotional and Spiritual Needs
The AdventHealth Hospice Care East Florida team is comprised of professionals who provide comprehensive medical, spiritual, psycho social and compassionate end-of-life care to individuals of all faiths throughout Volusia and Flagler counties. Treatment is focused on the person — not the disease — and to help and support the family, as well as the individual.
Each team member plays a role in meeting the physical, emotional and spiritual needs of patients, their families and daily caregivers.
Team Members
Hospice Medical Director — The Medical Director coordinates patient care with the attending physician and is available for consultation.
Attending Physician — A vital member of the team who manages the patient’s care.
Hospice Nurse — Coordinates the individualized plan of care.
Home Health Aide — Assists patients to help perform personal care, errands and light housekeeping.
Volunteer — Provides different types of support to patients and their loved ones, volunteers are trained and specially selected to help with transportation, running errands, preparing light meals, staying with a patient to give family members a respite, lending emotional support/companionship to patients and family members, and may offer bereavement support.
Chaplain — Visits and provides non-denominational, spiritual support throughout the illness and assists with funeral services if needed.
Social Worker — Helps the caregiver through emotional support, counseling and connections to community resources, such as financial assistance, transportation, funeral arrangements and government benefits.
Bereavement Counselor — Provides support to the patient, family grief education and support for 13 months following death.
