Meds, 24-Hour Service, Palliative Care, and Directives To Care For The Patient and Caregivers.
AdventHealth Hospice Care East Florida provides the medication, equipment and supplies necessary to maintain comfort and control symptoms as determined by the Interdisciplinary Team, whether the patient is receiving care at one of our inpatient facilities, a local nursing home, assisted living facility or while receiving in-home care.
24-Hour On-Call Service
AdventHealth Hospice Care East Florida staff are available on call after the administrative office has closed, seven days a week, 24 hours a day. Nurses, Chaplains, Social Workers and an Administrator are available on call as well. Typically, the patient and caregiver are given a telephone number to call to either speak to a nurse or leave a call-back number.
Advanced Care Planning
AdventHealth Hospice Care East Florida can help patients and their families with Advanced Care Planning and making decisions about the healthcare a person would want to receive if he becomes unable to speak for himself.
Such planning can include:
- Obtaining information on the types of life-sustaining treatments available.
- Deciding which types of treatment are preferred or desired during a life-limiting illness.
- Sharing personal values with loved ones.
- Completing an advanced directive to put into writing information regarding these decisions, and to identify who is able to speak for you if you are unable to speak for yourself.
Hospice care is covered by almost all insurance providers.
